Management Team
The Management Team provides the leadership and directs the operation of the Primary Care Network (PCN). This includes setting strategic direction, delivering innovation, development and working in collaboration to deliver shared goals. The Clinical Directors are supported by two operational roles of the PCN Manager and Transformation Lead.

Dr Thakrar is a lead partner at Acre and Acrefield Surgeries. He was appointed to the role as Clinical Director of the PCN at its formation in 2019.
He is experienced in all areas of general practice and has keen interest in Cardiology. He works one day a week in Hillingdon Hospital in the cardiology department and is the Clinical Cardiology lead for the local authority. He enjoys general practice and can also do joint injections for shoulder problems.

Primary qualifications: MB ChB, MRCGP Liverpool University Medical School 1984.
Martin is the lead partner at Devonshire Lodge Practice in Eastcote, joining in October 1989. He was appointed as joint Clinical Director of the PCN in 2021 when his practice joined the PCN. Martin is the PCN lead for Mental Health.

Ruby joined the PCN in MM/YY when the role of PCN Manager was established.
Ruby oversees the delivery of the PCN services and manages the growing PCN workforce.

Helen has worked with the PCN for 3 years on transformation projects, joining them full time in April 2023.
Helen’s background is in project management and she helps the PCN with improvement projects and developing the Primary Care Network.
Pharmacists
Our pharmacists are working in close association with the GP surgeries. They perform structured medication reviews, helps to reduce polypharmacy, undertakes activities which reduces medication related side effects. They play a key role in reducing medication related errors. They also help the practice in effective management of chronic diseases. Some of them run special clinics and see patients face to face as well. Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring.

Care Coordinators support with the organisation of a patient’s care. This could be anything from making appointments, organising and maintaining patient records, directing patient enquiries or even running some services such as blood pressure checks.

Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. Mental Health Practitioners
Our Mental Health Practitioner works with wider services to support patients who are leaving mental health care at hospital and returning to the care of their GP or who need to be referred into hospital services. This is often referred to as ‘Step up’ or ‘Step down’ support and aims to help patients navigate mental health services.
